Congratulations on your decision to have your child baptized at Our Lady of Perpetual Help Catholic Church. For those who are planning to have their child baptized, please read below on how to proceed:
• Please call the parish office, 808-689-8681 Ext. 107, to pick up a Baptism packet or for one to be mailed/emailed to you.
• Complete the application for baptism. Godparents must also read the requirements for godparents, then sign it
• Call Julia at 689-8681 Ext. 107 for a meeting when all paper works are done
• Interview with clergy may be required
• Mandatory preparation classes for parents and godparents. Please call Julia at 808-689-8681 Ext. 107 for schedule of class(es).

Requirements for Godparents:
• Must have completed sixteen years of age
• Fully initiated Catholic having received the sacraments of Baptism, Confirmation and Eucharist
• If married, marriage must be in the Catholic Church
• If single, not cohabitating, meaning living with someone but not married, and again, marriage must be in the Catholic Church
• Able and willing to take the role and responsibilities of a godparent/sponsor
Several notes on the Sacrament of Baptism:
- Infant Baptism is for children below the age of seven. Children 7 years old and above will go through the process of the Rite of Christian Initiation. They attend religious education classes on Sundays.
- Baptism in the Catholic Church means the child will be raised in the Catholic faith
- At least one parent must be Catholic and the non-Catholic parent agrees that the child will be raised in the Catholic faith
- Catholic parent(s) must start living the catholic faith, if they have not been doing so. They have to pass on the Catholic faith to their child/children by living it.
- One godparent, either male or female, is enough but if there are two, then they must be a male and a female -- not both males or both females.
